This ad or others like it were found in the Famous Monsters of Filmland magazine during the 1960s. The back pages of that publication (the abnormal brainchild of Forrest J. Ackerman, the original fanboy) were filled with ads for all kinds of curios and grotesqueries offered mail order from the Philadelphia-based outfit Captain Company.
